How do I file a claim or appeal?
If you wish to file a claim for medical, dental, vision, and weekly disability (time loss) benefits or if you are dissatisfied with an eligibility determination, benefit denial, or partial benefit award or any other adverse benefit determination by the Trust, you must follow the appropriate procedures.
